CANCELLATION POLICY:

NO REFUNDS will be allowed for any reason except for programs cancelled by
Rick Heinz Hockey Schools, or unless Insurance is purchased (see details
below).

R.H.H.S. RESERVES THE RIGHT TO CANCEL ANY SCHOOL AT ANYTIME DUE TO LOW
REGISTRATIONS - IF A SCHOOL CANCELLATION OCCURS YOU WILL BE NOTIFIED - (you
will have the option to transfer to another school or receive a full
refund). There are no reduction in fees for late arrivals, early
departures or illness/injuries to OR from the goalie school.

TERMS OF INSURANCE:

WE HAVE A NO REFUND POLICY - BUT we are now offering a non-refundable
insurance which will be $25.00.

DETAILS: R.H.H.S. must be in receipt of your cancellation notification one month
before school start date, in order to receive a full refund (amount
paid for the school) less an additional $50.00 cancellation fee.
Cancellation notification that is NOT received within 30 days of the school
start date will receive a refund (amount paid for the school) less an
additional $125.00 ($25.00 cancellation fee and $100.00 late notice fee).
If cancellation occurs after the school begins the fee will be $125.00 plus
cost for each day attended.

PLEASE READ THIS CAREFULLY BEFORE ACCEPTING:

The participant agrees that Rick Heinz Hockey Schools, our instructors and
the skating rink will not be held responsible for any accidents, injury or
loss, however caused, and agrees to release the proprietors and/or skating
rink from all claims which may arise as a result of or any reason of such
accidents, injury or loss. I also authorize Rick Heinz Hockey Schools to
charge my credit card a pre-paid , non-refundable registration fee for the
person named above, with receipt of this application by way of telephone,
fax, writing, or internet. I am also fully aware of the school insurance
and that all monies paid are non-refundable.
